What is an Inclusion and Intervention Plan?

An Inclusion and Intervention Plan (IIP) is a written document developed and implemented by a collaborative team. It is an individual plan that describes supports, strategies and interventions to optimize student learning in an inclusive school setting. The IIP includes:

  • student identification and background information;
  • a holistic summary of who the student is in terms of that student’s strengths, interests and learning styles;
  • relevant assessment information to identify the student’s needs and guide planning;
  • prioritized areas of development for the school year. Areas of development may include independence; personal/social well-being; communication; health/medical need/personal care; academic achievement; safety; sensory; motor skills and transition;
  • specific, measurable outcomes for the prioritised areas of development;
  • documentation of strategies and resources to assist the student in successfully achieving his/her individualized learning outcomes;
  • team members who will implement the strategies outlined in the IIP;
  • plans for assessment and review of progress on learning outcomes;
  • plans for both short-term and long-term transitions; and,
  • signatures of team members.

How are parents/caregivers involved in the Inclusion and Intervention Plan?

Parents and caregivers are members of the collaborative team. Parents/caregivers are encouraged to be involved in the process of creating and reviewing the IIP for their child.

How are students involved in the Inclusion and Intervention Plan?

The IIP team members are encouraged to help the student understand and participate in the IIP process in a way that is appropriate for the student’s level of understanding.

How is an Inclusion and Intervention Plan developed?

The Ministry of Education has developed an IIP planning template that is available electronically through the provincial Student Data System (SDS). This electronic Inclusion and Intervention Plan (eIIP) can be completed by the collaborative team at an IIP planning meeting. Access to the SDS is available by contacting the Coordinator of Student Achievement and Supports. A SDS authorization form must be filled out on an annual basis to have access. Your username and password will be provided by the division office. You can access the SDS by clicking here. Once the information has been entered into the eIIP, a Comprehensive Report can be printed for team members to sign and keep on file as a hard copy of the IIP. This report can be updated by the team throughout the school year and is easily transferred to a new school or school division within Saskatchewan if the student changes schools. A hard copy of the IIP is provided to the Coordinator of Student Achievement & Supports in September of each year.

Progress Reports

Progress reports are completed in the SDS on the same cycle as progress reports. The progress report includes an option to assess each outcome that has been written for the student using a 1-4 rating scale, comments section, and next steps toward the outcome. It is essential that the next steps toward the outcome are identified each term. The IIP progress report is provided to parents with the student progress report each term. Parents, classroom teachers, and principals sign the progress reports at student led conferences. A copy of the end of June progress report for the IIP is submitted in hard copy to the Coordinator of Student Achievement & Supports.
