Transitions for Students Who Require Intensive Supports

When a student who requires intensive supports enters a school it is best practice for the school, in conjunction with the Coordinator of Student Achievement & Supports, to facilitate a transition meeting with parents and inter-agency partners involved.

At the Pre-Kindergarten level the transition guide entitled “Moving on In” is available to parents, school staff, and interagency partners from either school division, Sunrise Children’s Therapies, Kids First, Parkland Early Childhood Intervention Program (PECIP), or Families First – Kamsack. This guide should be used to structure the meeting and ensure that the appropriate partners are involved.

This guide should be used to structure the meeting and ensure that the appropriate partners are involved.

When a student who requires intensive supports registers in Grade 1-12 a transition meeting with the sending school (when possible) and outside agency partners (if applicable) should occur. If safety considerations or specialized equipment/programming are needed, please arrange for the appropriate supports to be in place before the student begins school. The student services staff is in process of developing a meeting template and procedure guide.
