Student Medical Form – Every student should have this form filled out by their parents at the start of the school year in order to have an account of any medical conditions your child may have before partaking in any extra-curricular or school excursion activity.

Lunch Hour Contract – This form needs to be filled by parents if their child wants to leave school grounds during the noon hour.


All registration forms require the latest version of Adobe Reader to view. You can download Adobe Reader for free by clicking here.

To fill out the form, you will have to download the form first by right clicking on the link and then click on “Save Link as…” and choose where you want to save it. Then open it using Adobe Reader. After you have entered all your information you can then save the PDF form and email it to or you can print it out and drop it off at the school.

If you have Google Chrome, and if you have installed the Adobe Reader plugin in Google Chrome, you can open and save that way
