REGISTRATION

Parents wishing to register their children as students at St. Alphonsus are asked to fill in the applicable registration form:

Kindergarten Registration 2023-24 - March 7-2023

Authorization to Release Records Form (fillable)

301.1 Non-Catholic Student Declaration of Intention Form (fillable)

140.1 Permission Form for Use of Student Work and Personal Information (fillable)

Should you require a Student Supplies List, they can be found under the “Parents” tab in the top menu.

All registration forms require the latest version of Adobe Reader to view. You can download Adobe Reader for free by clicking here.

To fill out the form, you will have to download the form first by right clicking on the link and then click on “Save Link as…” and choose where you want to save it. Then open it using Adobe Reader. After you have entered all your information you can then save the PDF form and email it to saint.alphonsus@cttcs.ca or you can print it out and drop it off at the school.

If you have Google Chrome, and if you have installed the Adobe Reader plugin in Google Chrome, you can open and save that way.

Forms can also be faxed to (306)782-3077, or you can mail the completed forms to the school.
